Welcome to the Metaverse Justice Group's Online Estate Planning Questionnaire
Registration Please use your legal name as it appears on your license or government-issue ID
Total Estate Value The value of your total estate includes all assets held either in your individual name, jointly, or that you control. Assets can include but are not limited to real estate, your retirement accounts, any bank accounts, brokerage accounts, life insurance, business interests, and valuable jewelry and collectibles. The value of the assets goes by either the amount of money held in the account or the fair market value of any real estate, tangible property, or intangible asset (e.g. business interest)
Your Contact Details If you are married, your spouse will be defaulted as your primary agent throughout this interview. Please know that you are not required to have your spouse as your primary agent and you may choose to have another individual act as your primary helper.
Your Spouse Information Please use your legal name as it appears on your license or government-issue ID
Guardians for Your Minor Children It is important to put in your Will who you prefer to serve as the Guardian of your minor children. A Guardian is a person who is responsible for the day to day needs of a minor child and who steps into your role of being the parent if you are no longer living or unable to care for your child. A surviving spouse or the surviving biological parent is typically named as the first guardian though you may choose a different order depending on the circumstances. If you choose to have your Guardians serve jointly, your Guardians will have to make decisions together by majority decision. In choosing whether your Guardians should serve jointly, you should consider whether your Guardians would be able to get along and make decisions together. If you select that you would like your Guardians to serve jointly, you will want to select the “serve jointly” button next to each Guardian who you wish to have serve jointly. If you wish to have only two Guardians serve jointly and another Guardian serve individually, please only select the box next to the two joint Guardians and leave the box unselected next to the Guardian you wish to serve individually.
Beneficiary Information In deciding how to distribute your assets, please note that if you are married, we assume that you will leave all of your assets to your spouse and then your children (if applicable). In this section, you are being asked to designate a beneficiary to receive your assets if you are not married at the time of your death and have no children. Your beneficiary may be an individual(s) or a charity. If you decide to leave assets to a charity, please provide the full name and location of the charity. NOTE: Your beneficiary will only receive assets that pass through your Will. Assets that have named beneficiaries, such as life insurance policies, retirement accounts, annuities, etc., will be distributed according to the beneficiary forms you completed.
Executor-Spouse/Partner Need Help?
• Executor / Personal Representative: Administers your estate, ensures debts and taxes are paid, and distributes assets according to your Will.
• Primary Executor: If you are married, your spouse is the default Primary Executor. This field may be overwritten if necessary.
• Successor Executors: Individuals who will serve if the Primary Executor cannot act.
• Serve Jointly vs Individually:
• Jointly: Executors make decisions together. Choose this only if they can cooperate effectively.
• Individually: Executors act independently without needing joint approval.
• Important Considerations: Select responsible, reliable individuals. If Executors serve jointly, they should be able to work well together.
